INTRODUCTION

Welcome to Coco Med Spa's Beauty Bank Program. This document outlines the terms and conditions governing your membership and participation in our Beauty Bank Program. By enrolling in the Beauty Bank Program, you acknowledge that you have read, understood, and agree to be bound by these terms and conditions.

MEMBERSHIP PLANS

Coco Med Spa currently offers two Beauty Bank membership plans:

  1. ESSENTIALS: $100/month

  2. LUXE: $250/month

MEMBERSHIP BENEFITS

ESSENTIALS Membership ($100/month)

  • The $100/month you pay will be banked and can be used towards any service

  • 10% off retail products

  • 2 B12 injections per month

  • Complimentary birthday facial

LUXE Membership ($250/month)

  • The $250/month you pay will be banked and can be used towards any service

  • 10% off retail products

  • 10% off toxins (Botox, Dysport, etc.)

  • 4 B12 injections per month

  • Complimentary birthday facial

PAYMENT TERMS

  1. Monthly Fees: Members agree to pay the monthly fee for their selected membership plan ($100 for ESSENTIALS or $250 for LUXE).

  2. Billing Date: You'll be automatically charged on the 1st of each month with the card you have on file.

  3. Payment Method: A valid credit card or payment method must be kept on file for automatic monthly charges.

  4. Failure to Process Payment: If your payment cannot be processed, Coco Med Spa reserves the right to suspend membership benefits until payment is received.

BEAUTY BANK FUNDS

  1. Fund Accrual: Your monthly membership fee is deposited directly into your personal Beauty Bank account.

  2. Fund Usage: Beauty Bank funds can be used towards any service offered at Coco Med Spa.

  3. No Expiration: Funds in your Beauty Bank do not expire, even if you cancel your membership. They will remain as a gift card on file that can be used for any service at any time.

  4. Balance Inquiries: To check your Beauty Bank balance, you may text or call 727-954-6166.

REFUND POLICY

  1. Non-Refundable Deposits: All Beauty Bank contributions are considered non-refundable deposits once they have been added to your Beauty Bank balance.

  2. Service Credit: Rather than providing cash refunds, Beauty Bank funds remain available as service credit that can be used for any service at Coco Med Spa.

  3. Exceptional Circumstances: Refund requests due to exceptional circumstances (such as medical conditions preventing use of services, relocation outside of service area beyond 50 miles, or other extenuating circumstances) may be reviewed on a case-by-case basis at the sole discretion of Coco Med Spa management.

  4. Refund Processing: If a refund is approved under exceptional circumstances, a processing fee of 5% may be deducted from the refunded amount.

  5. Partial Usage: If some portion of the Beauty Bank funds has already been used, only the remaining unused balance may be considered for a refund under exceptional circumstances.

MEMBERSHIP BENEFITS

  1. Discounts: Members receive specified discounts on services and products as outlined in their membership tier.

  2. B12 Injections: Members receive a specified number of complimentary B12 injections per month based on their membership tier.

  3. Birthday Facial: All members receive a complimentary birthday facial.

  4. Benefit Eligibility: Exclusive discounts are only available to active members. If you cancel your membership, you will lose the benefits of exclusive discounts while retaining your accumulated funds.

CANCELLATION POLICY

  1. Cancellation Request: You may cancel at any time within the 30-day period prior to your next monthly charge with no penalty.

  2. Notice Period: To cancel, you must provide notice at least 30 days before your next billing date.

  3. Remaining Funds: Upon cancellation, your existing savings will remain on file ready for use and do not expire. However, you will lose access to member-exclusive discounts as they are only available to active members.

  4. Cancellation Method: To cancel your membership, please call 727-954-6166.

MEMBERSHIP CHANGES

  1. Plan Changes: Members may upgrade or downgrade their membership plan by contacting Coco Med Spa.

  2. Effective Date: Changes to membership plans will take effect on the next billing cycle.

APPOINTMENT POLICIES

  1. Booking: Members are encouraged to book appointments in advance to ensure availability.

  2. Cancellation: Appointments must be canceled at least 24 hours in advance to avoid a cancellation fee.

  3. No-Shows: Failure to appear for scheduled appointments may result in a fee or the deduction of Beauty Bank funds equivalent to the service value.

MODIFICATIONS TO TERMS

Coco Med Spa reserves the right to modify these terms and conditions at any time. Members will be notified of any changes, and continued membership after such notification constitutes acceptance of the modified terms.

GOVERNING LAW

These terms and conditions shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions.
